Bipolar disorder vs borderline personality disorder

The short answer
These two are confused often enough that researchers have built machine-learning models to try to tell them apart. That effort is itself the most useful evidence available: across five studies covering 591 participants, the models were considerably better at identifying bipolar disorder than borderline personality disorder. The harder half of the problem is BPD, and the authors read that as reflecting a real ongoing difficulty in diagnosing it.
Diagnostic disagreement is not peculiar to this pair. When 1,038 psychiatrists and physicians across 19 countries read the same standardised cases, they landed on the same diagnosis roughly 55% of the time, and the confusions were systematic rather than random, which points at ambiguity in the diagnostic categories themselves rather than carelessness.
In broad terms, bipolar disorder is defined by episodes of elevated and depressed mood lasting days to weeks, while borderline personality disorder is defined by a persistent pattern of instability in mood, self-image and relationships, with shifts that tend to be shorter and more often tied to what is happening between people. The first solid genetic map of borderline personality disorder
The largest genetic study of borderline personality disorder so far compared 12,339 people with BPD to 1,041,717 people without it. It found 11 regions of the genome linked to the disorder, with common variants accounting for about 17.3% of underlying risk.

Common questions
Why is borderline personality disorder harder to identify than bipolar disorder?
In the review we covered, machine-learning models were far more accurate at identifying bipolar disorder than BPD. The authors treat that gap as evidence of a genuine difficulty in diagnosing BPD rather than a limitation of the models.
How accurate were the models?
Models built on brain-imaging features reached about 80% accuracy, and smartphone-based mood data distinguished bipolar disorder, BPD and healthy controls with about 75% accuracy. The authors are explicit that more research is needed before any of this is usable clinically.
How often do psychiatrists disagree about a diagnosis?
In a study of 1,038 clinicians across 19 countries reading nine standardised written cases, inter-rater agreement was modest, working out to the same diagnosis roughly 55% of the time, with accuracy against a reference standard averaging about 66%. Schizophrenia cases were the hardest.
Can someone have both?
