The BPD brain is less flexible — and more variable

This study examined how the brains of women with BPD function over time, rather than just how they look at rest. Using resting‑state fMRI, researchers compared 47 women with BPD to 28 healthy controls and analyzed how their brain networks shift, transition, and reorganize. They found that people with BPD show both reduced flexibility in how their brain networks move between states and an unusually wide range of network configurations overall. Greater emotion‑regulation problems were linked to a larger variety of brain states, while fewer transitions between states were tied to more severe BPD symptoms, more childhood trauma, and higher dissociation. Trauma history and emotion‑regulation difficulties also shaped how time spent in certain brain states related to symptom severity. Overall, the findings suggest that altered brain dynamics—less flexibility but more variability—may underlie core BPD features, especially those connected to early trauma and emotion dysregulation.
