Finding the brain basis of BPD identity disturbance

Researchers studied identity disturbance as early as possible — before medication or comorbidity could muddy the picture.
Researchers wanted to find a measurable brain basis for one of BPD's defining features: a disturbed sense of identity, by studying it as early as possible, before medication or other psychiatric conditions could complicate the picture. They scanned 27 medication-naïve adolescent girls with BPD and no other psychiatric diagnoses, alongside 28 matched healthy peers, using fMRI while participants answered true-or-false questions about themselves, about an acquaintance, or about neutral facts. When comparing self-reflection to simple fact-processing, the BPD group showed markedly reduced activity in the right dorsolateral prefrontal cortex, a region tied to top-down cognitive control, along with weaker activity in some visual and parietal regions. This pattern was specific to thinking about themselves; when reflecting on another person, the difference showed up in a completely different brain region, the subgenual anterior cingulate cortex, and turned out to reflect something different: a failure to properly "quiet down" that region during the unrelated fact-processing task, rather than a problem with other-reflection itself. In healthy adolescents, self- and other-reflection typically activate very similar brain networks. The fact that BPD patients showed two distinct breakdowns in two different regions suggests these two processes might become oddly decoupled from each other in the disorder. The researchers interpret the main finding, the prefrontal cortex dip during self-reflection, as a sign of weakened top-down regulation when processing identity-related information, giving an early, measurable neurobiological clue to where BPD's hallmark identity disturbance might originate.
